Step 1: Complete a Patient Information & Referral Form
To enable your Patient coordinator to select the proper hospital and specialist for you, we will ask that you complete a Patient Information & Referral Form (Please E-Mail the completed Form to: ueliwanner@slh.ch)

Step 2: Medical Report, Films and Pathology Slides
After submitting the Patient Information Form via email, we will provide you with courier instructions to send any relevant medical records or physician reports (in English), films (X-ray, MRI, CT scan, ultrasound), and pathology reports as applicable.

Step 3: Appointment Confirmation
Once the Patient Information Form and your medical information are received, your Patient
Coordinator will confirm the date and time of your appointment at your chosen hospital.
